245D Waiver Services

Basic & Intensive Support Services through the 245D Home & Community Based Services.

PDS offers Basic & Intensive Support Services through the 245D Home & Community Based Services. PDS direct support staff are specifically trained to provide services to disabled and senior individuals in a community setting. All direct support staff are required to use electronic visit verification (EVV).

Person-Centered Approach

At PDS, 245D services are centered on each person's strengths, choices, needs, and goals. We provide the right level of support while promoting independence, dignity, safety, and meaningful community participation. Services are tailored to each person and adjusted as their needs and goals change.

Cost & funding

Services are funded through Medical Assistance (Medicaid) waivers and county programs — no out-of-pocket cost for eligible clients.

Basic Services

Basic 245D services support daily living, safety, and caregiver relief in home and community settings.

Individualized Home Supports (IHS) Without Training — Supervision, guidance, cueing, and assistance with daily tasks at home or in the community.
Homemaker Services — Light housekeeping and laundry; meal prep and shopping for groceries, clothing, and supplies; simple repairs, arranging transportation, and assistance with activities of daily living.
Respite (In‑Home & Out‑of‑Home) — Temporary care to give the main caregiver a break, short- or long-term, including overnight stays.
Adult Companion Services — Non‑medical social support and skill-building such as coping, community integration, and light supervision.
Night Supervision Services — Overnight monitoring and assistance with ADLs/IADLs, safety, and skill reinforcement.
Individual Community Living Supports (ICLS) — For adults 65+: reminders, cues, moderate supervision, adaptive skills, household tasks, health/safety, and community engagement.
24‑Hour Emergency Assistance — On-call problem-solving and immediate response when urgent support is needed.

Intensive Services

Intensive 245D services build independence, employment readiness, and community living skills.

Individualized Home Supports (With Training / Remote / With Family Training) — Skill-building to support daily living, health, community participation, and independence, offered in-person or remotely.
Supported Living Services (SLS) — Ongoing, personalized help with managing everyday tasks and living more independently.
Semi‑Independent Living Skills (SILS) — Support for people who can live on their own with some guidance, focusing on budgeting, cleaning, meal prep, and social skills.
Prevocational Services — Helps individuals build general work skills to prepare for future jobs with competitive pay.
Employment Exploration Services — Helps individuals learn about job options in their community through hands-on experiences, tours, and job shadowing.
Employment Development Services — Supports individuals in developing the skills and connections needed to pursue competitive employment.
