Mental Health

Adult Rehabilitative Mental Health Services (ARMHS)

ARMHS supports adults living with mental illness in developing everyday skills to live independently, manage their mental health, and stay connected to their community. Services focus on stability, recovery, and preventing hospitalization or more restrictive placements.

Families

Family Navigation (Family Tenancy Support Services — Hennepin County)

Referred through Hennepin County's Coordinated Entry System (CES), Family Navigation supports families experiencing homelessness and struggling with housing instability. Families are matched to one of four housing options: Minnehaha Townhomes, Hook & Ladder, Scattered Sites, and Emergency Housing Vouchers.

Disability Benefits

PDS SOAR Services

PDS is contracted through the MN Department of Human Services (DHS) to help Minnesotans access Supplemental Security Income (SSI) and Social Security Disability Income (SSDI). The SOAR Program (SSI/SSDI Outreach, Access, and Recovery) helps adults experiencing homelessness or at risk of homelessness apply for Social Security disability benefits.

PDS Problem Gambling Treatment

Problem gambling can disrupt your life, relationships, and emotional well-being. PDS is a certified problem gambling provider through the Minnesota Department of Human Services (DHS). An estimated 56,000 Minnesota adults meet criteria for problem gambling and about 162,000 more are considered at risk (Minnesota Department of Human Services, Problem Gambling Program) — gambling addiction is a serious public health issue, and help is available.

Conditional Release Support Services

PDS offers culturally responsive, trauma-informed, and person-centered support for individuals on Conditional Release Supervision through Hennepin County Department of Corrections & Community Rehabilitation (DOCCR). Our goal is to help clients meet court requirements, build stability, and reduce the risk of reoffending while supporting long-term success.
